A Clinical Surgical Thyroid Nodule

Abstract
During the 4 years period from Aug. 1980 through July 1983, authors experienced 125 cases of thyroid nodule by surgical intervention at Department of Surgery, St. Vincent Hospital, Catholic Medical College.
Here we represent the result of cases and brief review of thyroid nodule.
1) The sex distribution of thyroid nodules disclosed preponderance of female with the ratio of 1: 6. 4
2) The peak incidence of thyroid nodules was noted in the 4th decade (33.6%) and the majority of cases (83.2%) were in 3rd and 5th decade.
3) The incidence of carcinoma in thyroid nodules were 17.6% in male and 19.4% in female and peak incidence was noted in the age of 60 years or more (50%).
4) The pathologic classification of thyroid nodules was nodular hyperplasia 40 cases (32%), adenoma 36 cases (28.8%), cyst 16 cases (12.8%), thyroiditis 9 cases (7.2%), carcinoma 24 cases (19.2%).
5) The pathologic classification of thyroid carcinoma was papillary carcinoma in 18 cases (85%), follicular carcinoma in 4 cases (16.7%), undifferentiated carcinoma and metastatic carcinoma 1 case (4.2%) respectively.
6) The most prominent symptoms and signs were palpable mass on ant. neck, palpitation, fatigue, wt. loss, neck pain, dyspnea in order of frequency. In 24 cases of malignancy, 4 cases have already metastased to adjacent lymph nodes.
7) In 119 cases of thyroid scanning, 103 cases revealed cold nodule, of which 18 cases (17.4%) were confirmed as thyroid carcinoma at later.
8) In the majority of case, the duration of illness within a year (57.4%).
9) In benign nodules, we performed?unilat?lobectomy 53 cases (52.4%), subtotal lobectomy 21 cases (20.8%), subtotal thyroidectomy 19 cases (18.8%). And unilat. lobectomy 8 cases (33.3%), subtotal thyroidectomy 6 cases (25.0%), near total thyroidectomy 4 cases (16.7%), modified RND 4 cases (16.7%) were performed in malignancy.
10) Postop. complications occurred as follow; hematoma 6 cases (4.8%), transient hoarseness 6 cases (4.8%), wound infection 3 cases (2.4%)
